Common use
Tiova Rotacap is indicated for the long-term, once-daily maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD). It is used in patients with a confirmed diagnosis of COPD, including those with chronic bronchitis and/or emphysema, to help control symptoms, improve lung function, and reduce the frequency of flare-ups. It is not indicated for the initial treatment of acute bronchospasm or for use in asthma.
Dosage and direction
The recommended dosage for Tiova Rotacap is the inhalation of the contents of one capsule (18 mcg) once daily, at the same time each day, using the Rotahaler® device. It should not be used more than once in 24 hours. To use, open the blister pack, place the capsule in the Rotahaler®, pierce the capsule by pressing the side buttons firmly once, exhale fully away from the device, place the mouthpiece between the lips, and inhale deeply and steadily. Hold the breath for up to 10 seconds if possible, then remove the device and exhale slowly. The used capsule may be discarded; the device should be kept clean and dry.
Precautions
- Not for use as a rescue medication for acute bronchospasm; ensure patients have a short-acting bronchodilator available.
- Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction due to anticholinergic effects.
- May cause paradoxical bronchospasm; if this occurs, discontinue use immediately and consider alternative therapy.
- Avoid getting the powder in the eyes, as it may cause blurring of vision or pupil dilation.
- Not recommended for use in patients with severe renal impairment (creatinine clearance ≤50 mL/min), unless the potential benefit outweighs the risk.
- Monitor for signs of urinary retention, especially in patients with pre-existing urinary conditions.
Contraindications
Tiova Rotacap is contraindicated in patients with a known hypersensitivity to tiotropium bromide, atropine or its derivatives, or any component of the formulation. It should not be used for the treatment of asthma, as safety and efficacy have not been established in this population. It is also contraindicated in patients with a history of hypersensitivity reactions such as rash, urticaria, or angioedema to similar anticholinergic agents.
Possible side effects
Common side effects may include dry mouth, constipation, upper respiratory tract infection, sinusitis, pharyngitis, and non-specific chest pain. Less commonly, patients may experience urinary retention, blurred vision, glaucoma, tachycardia, palpitations, and paradoxical bronchospasm. Allergic reactions such as skin rash, itching, or swelling of the face/lips/tongue are rare but possible. If any severe or persistent side effects occur, medical attention should be sought.
Drug interaction
Concomitant use with other anticholinergic-containing drugs (e.g., ipratropium, aclidinium) may potentiate anticholinergic adverse effects and is not recommended. Use with sympathomimetic agents (e.g., beta-agonists) may enhance bronchodilation but also increase the risk of cardiovascular effects. Caution is advised when using with drugs that are excreted renally or that may affect renal function. No clinically significant interactions have been observed with cytochrome P450 substrates.
Missed dose
If a dose is missed, it should be taken as soon as remembered on the same day. However, if it is almost time for the next dose, the missed dose should be skipped and the regular dosing schedule resumed. Do not double the dose to make up for a missed one. Consistent daily use is important for optimal effect, so patients should try to adhere to the prescribed schedule.
Overdose
Overdose may lead to exaggerated anticholinergic effects such as dry mouth, visual disturbances, tachycardia, urinary retention, and constipation. In severe cases, agitation, confusion, or cardiac arrhythmias may occur. There is no specific antidote; treatment should be symptomatic and supportive. Medical attention should be sought immediately if overdose is suspected. Dialysis is not likely to be effective due to the low systemic absorption and high protein binding of tiotropium.
Storage
Store below 30°C, in a dry place, protected from light and moisture. Keep the capsules in the original blister packaging until immediately before use to protect them from humidity. Do not store in the bathroom or near sinks. Keep out of reach of children and pets. Do not use capsules that are discolored, damaged, or that have been exposed to excessive moisture.
