Common use

Dosage and direction

The recommended starting dose for erectile dysfunction is 10 mg taken prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on efficacy and tolerability. Tadalafil may be taken without regard to food. For daily use, a dose of 2.5 mg or 5 mg taken at approximately the same time each day is recommended, allowing for spontaneous sexual activity at any time. The maximum recommended dosing frequency is once per day. For benign prostatic hyperplasia, the recommended dose is 5 mg taken once daily at approximately the same time every day. The dosage must be strictly adhered to as prescribed; do not adjust without consulting your physician.

Precautions

Prior to taking Tadalafil, a thorough medical evaluation should be conducted to assess cardiovascular status, as sexual activity carries potential cardiac risk. Contraindications

Tadalafil is contraindicated in patients using any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) regularly or intermittently, or guanylate cyclase stimulators (e.g., riociguat). Concomitant use can lead to a sudden and dangerous drop in blood pressure. It is also contraindicated in patients with a known hypersensitivity to Tadalafil or any of the tablet’s excipients. History of non-arteritic anterior ischemic optic neuropathy (NAION) is a contraindication.

Possible side effect

The most common side effects include headache, dyspepsia, back pain, myalgia, nasal congestion, flushing, and limb pain. These are usually transient and mild to moderate in nature. Serious side effects, though rare, include priapism (a prolonged and painful erection lasting more than 4 hours), sudden hearing loss, sudden vision loss (NAION), and cardiovascular events (e.g., myocardial infarction, stroke, unstable angina). Allergic reactions, including skin rashes, are possible. If you experience any severe or persistent side effects, discontinue use and contact your doctor immediately.

Drug interaction

Tadalafil is primarily metabolized by CYP3A4. Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, clarithromycin) significantly increases Tadalafil exposure; a dose reduction is required. Alpha-blockers (e.g., doxazosin, tamsulosin) can potentiate the blood-pressure-lowering effects; concomitant use should be approached with caution and only under physician guidance. Avoid concomitant use with nitrates. Alcohol can increase the potential for orthostatic signs and symptoms, including increased heart rate, decreased standing blood pressure, dizziness, and headache.

Missed dose

For the as-needed regimen: If a dose is missed, take it when you remember, but ensure there is adequate time (at least 30-60 minutes) before sexual activity. Do not take more than one dose in a 24-hour period. For the once-daily regimen: If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and take your next dose at the regular time. Do not take a double dose to make up for a missed one.

Overdose

In cases of suspected overdose, standard supportive measures should be employed. The most likely expected symptoms would be a reflection of the drug’s vasodilatory properties, including severe headache, flushing, dizziness, and potentially significant hypotension. Hemodialysis is not expected to accelerate clearance as Tadalafil is highly protein-bound. In the event of an overdose, seek immediate medical attention or contact a Poison Control Center.

Storage

Store at room temperature (20°C to 25°C or 68°F to 77°F), with excursions permitted between 15°C and 30°C (59°F to 86°F). Keep the tablets in their original blister pack or bottle to protect from light and moisture. Keep out of the reach of children and pets. Do not use after the expiration date printed on the packaging. Properly dispose of any unused or expired medication.
