Common use
Sildamax is indicated for the treatment of erectile dysfunction, a condition characterized by the consistent inability to attain or sustain an erection adequate for satisfactory sexual performance. It is used on an as-needed basis and is not intended for continuous daily administration. The medication is suitable for men with ED of various etiologies, including vasculogenic, neurogenic, and psychogenic origins, and may be used in conjunction with lifestyle modifications or other therapeutic approaches under medical supervision.
Dosage and direction
The recommended starting dose of Sildamax is 50mg, taken approximately one hour before anticipated sexual activity. Based on efficacy and tolerability, the dose may be increased to 100mg or decreased to 25mg. The tablet should be swallowed whole with a glass of water; it may be taken with or without food, though a high-fat meal may delay onset of action. Do not take more than one dose within a 24-hour period. Sexual stimulation is required for the medication to be effective.
Precautions
- Not intended for use by women, children, or adolescents
- Use with caution in patients with cardiovascular disease; sexual activity poses a potential cardiac risk
- Avoid consumption of grapefruit or grapefruit juice, as it may increase sildenafil concentrations
- May cause hypotension; caution advised in patients taking antihypertensives or with volume depletion
- Priapism (prolonged and painful erection lasting more than 4 hours) requires immediate medical attention to prevent permanent damage
- Discontinue use and seek medical advice if sudden vision or hearing loss occurs
Contraindications
- Hypersensitivity to sildenafil or any excipients in the formulation
- Concurrent use of nitrates or nitric oxide donors in any form
- Patients with severe hepatic impairment
- Patients with hypotension (BP < 90/50 mmHg) or recent history of stroke or myocardial infarction
- Hereditary degenerative retinal disorders
- Concomitant use with riociguat
Possible side effects
Common side effects may include headache, flushing, dyspepsia, nasal congestion, dizziness, and visual disturbances (such as altered color perception). Less frequently reported effects include myalgia, back pain, and rash. Rare but serious adverse events include priapism, non-arteritic anterior ischemic optic neuropathy (NAION), sudden hearing loss, and severe cardiovascular events. Most side effects are mild to moderate and transient in nature.
Drug interaction
Sildamax is contraindicated with nitrates and riociguat. Concurrent use with alpha-blockers may lead to symptomatic hypotension. CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, erythromycin) can increase sildenafil plasma levels, while inducers (e.g., rifampin) may decrease them. Use caution with other antihypertensive agents. Alcohol may increase the risk of orthostatic hypotension and dizziness.
Missed dose
Sildamax is not intended for regular scheduled use. If a dose is missed, it may be taken when remembered, provided it aligns with the planned timing for sexual activity and at least 24 hours have passed since the last dose. Do not double the dose to make up for a missed one.
Overdose
In case of suspected overdose, seek immediate medical attention. Symptoms may include severe headache, visual disturbances, syncope, prolonged erection, and cardiovascular events. Standard supportive measures should be employed; haemodialysis is not expected to accelerate clearance. Management of priapism may require intracavernosal aspiration and alpha-adrenergic agonists.
Storage
Store below 25°C in the original packaging, protected from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
