Poxet: Clinically Proven Premature Ejaculation Management
Premature ejaculation (PE) is a prevalent male sexual health concern, impacting quality of life and intimate relationships. Poxet is a specialized oral medication formulated with the active ingredient Dapoxetine, a selective serotonin reuptake inhibitor (SSRI) specifically developed and approved for the on-demand treatment of premature ejaculation. Unlike conventional antidepressants used off-label, Poxet offers a rapid onset of action and a short half-life, making it an optimal choice for situational use. This product card provides a comprehensive, expert-level overview of Poxet, detailing its pharmacological profile, appropriate usage, and essential safety information for informed decision-making.
Features
- Active Ingredient: Dapoxetine Hydrochloride.
- Available Strengths: 30 mg and 60 mg film-coated tablets.
- Pharmacological Class: Selective Serotonin Reuptake Inhibitor (SSRI).
- Mechanism of Action: Rapid inhibition of serotonin reuptake, increasing serotonin activity in the synaptic cleft of the central nervous system, which is implicated in ejaculatory control.
- Onset of Action: Rapid absorption, designed to be taken 1-3 hours prior to anticipated sexual activity.
- Elimination Half-Life: Approximately 15-19 hours, shorter than daily SSRIs, minimizing long-term systemic exposure.
- Formulation: Oral tablet, designed for on-demand (prn) use rather than daily administration.
Benefits
- Significantly Increases Intravaginal Ejaculatory Latency Time (IELT): Clinical trials demonstrate a substantial, statistically significant increase in time to ejaculation.
- Improves Perceived Control Over Ejaculation: Users report a greater sense of command and timing during sexual activity.
- Enhances Sexual Satisfaction: For both the individual and their partner, leading to improved sexual experiences.
- Reduces Ejaculation-Related Personal Distress: Addresses the anxiety and psychological burden associated with premature ejaculation.
- On-Demand Dosing Flexibility: Does not require a daily medication regimen, aligning treatment with sexual activity.
- Evidence-Based Efficacy: Supported by robust, randomized, double-blind, placebo-controlled clinical studies.
Common use
Poxet is indicated for the treatment of premature ejaculation (PE) in adult men aged 18 to 64 years. A diagnosis of lifelong or acquired PE is typically based on the criteria of persistent or recurrent ejaculation with minimal sexual stimulation before, on, or shortly after penetration and before the person wishes it, which causes marked distress or interpersonal difficulty. It is intended for men who have a stable sexual relationship and are seeking to improve control over ejaculation. It is not indicated for use by women or adolescents.
Dosage and direction
The recommended starting dose is 30 mg, taken orally as a single dose approximately 1 to 3 hours prior to anticipated sexual activity. The tablet should be swallowed whole with at least a full glass of water; it should not be chewed or crushed. Based on efficacy and tolerability, the dose may be increased to a maximum recommended dose of 60 mg. Poxet should not be taken more than once within a 24-hour period. It can be taken with or without food; however, taking it with a full meal may reduce the potential for nausea. The need for continued treatment should be reassessed periodically.
Precautions
- Medical Evaluation: A thorough medical and psychiatric history should be obtained to rule out other causes of ejaculatory dysfunction before initiating treatment.
- Orthostatic Hypotension: Dapoxetine can cause dizziness, lightheadedness, and syncope due to orthostatic hypotension. Patients should be cautioned to rise slowly from a sitting or lying position.
- Mood Changes: Monitor for the emergence of anxiety, agitation, panic attacks, insomnia, irritability, hostility, aggressiveness, impulsivity, akathisia, hypomania, and mania. Patients with a history of mania/hypomania or bipolar disorder should generally not be treated with Poxet.
- Withdrawal: While the risk is lower than with chronic SSRI use due to its short half-life, abrupt discontinuation can potentially lead to symptoms such as dizziness, nausea, irritability, and sleep disturbances.
- Pregnancy and Breastfeeding: Although not used by women, male fertility does not appear to be affected. No special precautions are required for partners.
Contraindications
Poxet is contraindicated in patients with:
- Known hypersensitivity to Dapoxetine or any of the excipients in the formulation.
- Significant pathological cardiac conditions such as heart failure (NYHA Class II-IV), conduction abnormalities (e.g., sick sinus syndrome, Wolff-Parkinson-White syndrome), significant ischemic heart disease, or valvular heart disease.
- History of mania or severe depression.
- Concurrent administration with monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping an MAOI. Similarly, MAOIs should not be initiated within 7 days of stopping Poxet.
- Concurrent administration with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir, clarithromycin, telithromycin) or potent CYP2D6 inhibitors (e.g., fluoxetine, paroxetine).
- Concurrent administration with thioridazine or other drugs that prolong the QT interval.
Possible side effect
Like all medications, Poxet can cause side effects, although not everybody gets them. The most commonly reported adverse reactions are mild to moderate and often diminish with continued use. They include:
- Very Common (≥1/10): Nausea, dizziness, headache.
- Common (≥1/100 to <1/10): Diarrhea, insomnia, somnolence (sleepiness), fatigue, anxiety, vomiting, abdominal pain, dry mouth, hyperhidrosis (increased sweating), tremors, blurred vision, tinnitus.
- Uncommon (≥1/1,000 to <1/100): Syncope (fainting), orthostatic hypotension, palpitations, tachycardia, irritability, decreased libido, anorgasmia, erectile dysfunction, agitation, nervousness, confusion, attention disturbance, mydriasis (pupil dilation), vertigo.
- Rare: Priapism (prolonged and painful erection) has been reported with SSRIs and requires immediate medical attention.
Drug interaction
Poxet has the potential for significant interactions:
- Serotonergic Drugs: Concomitant use with other serotonergic agents (e.g., SSRIs, SNRIs, triptans, tramadol, tryptophan, St. John’s Wort) increases the risk of serotonin syndrome, a potentially life-threatening condition.
- CYP Enzymes: Dapoxetine is metabolized primarily by multiple CYP enzymes (CYP3A4, CYP2D6, CYP2C19, CYP2C9, and CYP2B6). Concomitant use with inhibitors of CYP3A4 (e.g., ketoconazole) or CYP2D6 (e.g., fluoxetine) is contraindicated as it significantly increases Dapoxetine exposure. Inducers of these enzymes (e.g., rifampicin) may decrease its efficacy.
- Alcohol: Patients should be advised to avoid alcohol while taking Poxet. Concomitant use can potentiate neurocognitive effects such as dizziness, drowsiness, and altered judgment, and increase the risk of syncope.
- Drugs Increasing Bleeding Risk: As with other SSRIs, Dapoxetine may increase the risk of bleeding, particularly when used concomitantly with NSAIDs (e.g., ibuprofen), aspirin, or warfarin.
- Alpha-1 Adrenergic Receptor Antagonists (e.g., tamsulosin): May potentiate orthostatic hypotension.
Missed dose
Poxet is intended for on-demand use only. There is no scheduled dosing regimen. If a dose is missed in relation to planned sexual activity, it should simply be omitted. The next dose should be taken 1-3 hours before the next anticipated sexual activity. Do not take a double dose to make up for a missed one.
Overdose
In cases of overdose, which may involve ingestion of multiple tablets, the symptoms are likely to be an extension of the known adverse reaction profile. Expected symptoms include dizziness, lightheadedness, nausea, vomiting, fatigue, somnolence, tachycardia, and syncope. Serotonin syndrome is a risk in significant overdose, especially if other serotonergic drugs are involved. There is no specific antidote for Dapoxetine overdose. Management consists of providing supportive care and treating symptoms. Gastric lavage and administration of activated charcoal may be considered if presented early. Close monitoring of cardiac function and vital signs is essential.
Storage
- Store below 30°C (86°F).
- Keep the tablets in their original blister pack to protect from light and moisture.
- Keep out of the sight and reach of children.
- Do not use after the expiration date printed on the packaging.
