Common use

Nexium is commonly used for the short-term treatment (4 to 8 weeks) of erosive esophagitis, for maintaining healing of erosive esophagitis, and for managing symptomatic gastroesophageal reflux disease (GERD). It is also indicated for reducing the risk of gastric ulcers developing in patients on continuous NSAID therapy, and as part of combination therapy for H. pylori infection. Some patients use the over-the-counter version for frequent heartburn (occurring 2 or more days per week).

Dosage and direction

The recommended adult dosage for healing erosive esophagitis is 20mg or 40mg once daily for 4 to 8 weeks. For maintenance of healing of erosive esophagitis, the dose is 20mg once daily. For GERD symptoms, 20mg once daily for 4 weeks is typical. For H. pylori eradication, Nexium 40mg once daily is combined with amoxicillin and clarithromycin for 10 days. Nexium should be taken at least one hour before a meal, preferably in the morning. The capsule must be swallowed whole; it should not be crushed, chewed, or split. For patients with difficulty swallowing, capsules may be opened and mixed with applesauce, or the oral suspension formulation may be used.

Precautions

Patients should inform their healthcare provider of any liver disease, as dosage adjustment may be necessary. Long-term use (beyond one year) may lead to decreased magnesium levels, vitamin B12 deficiency, or increased risk of bone fractures. Nexium may mask symptoms of gastric malignancy; appropriate diagnostic evaluation should be conducted if alarm symptoms (e.g., unintentional weight loss, recurrent vomiting, dysphagia) occur. Use during pregnancy only if clearly needed. Not recommended during breastfeeding unless benefits outweigh risks.

Contraindications

Nexium is contraindicated in patients with known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation. Concomitant use with rilpivirine-containing products is contraindicated due to the potential for reduced antiviral efficacy. Drug interaction

Nexium may alter the absorption of drugs that require gastric acid for bioavailability, such as ketoconazole, iron salts, erlotinib, dabigatran, and mycophenolate mofetil. It may increase systemic concentrations of drugs metabolized by CYP2C19, such as diazepam, citalopram, and imipramine. Concomitant use with clopidogrel may reduce the antiplatelet effect. St. John’s Wort or rifampin may decrease esomeprazole levels.

Missed dose

If a dose is missed, it should be taken as soon as remembered, unless it is nearly time for the next dose. In that case, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one.

Overdose

Limited data exist on esomeprazole overdose. Symptoms may reflect those of side effects but amplified, such as gastrointestinal disturbances, confusion, drowsiness, blurred vision, tachycardia, and sweating. Management is supportive and symptomatic; dialysis is not likely to be effective due to high protein binding. In case of suspected overdose, contact a poison control center or seek emergency medical attention.

Storage

Store at room temperature (20–25°C or 68–77°F), in a dry place, away from light and moisture. Keep in the original container with the lid tightly closed. Do not store in bathrooms or near sinks. Keep out of reach of children and pets. Do not use beyond the expiration date printed on the packaging.
