Dosage and direction

The recommended starting dose of Glycomet SR is 500 mg once daily, taken with the evening meal. Based on glycemic control and tolerability, the dose may be increased by 500 mg weekly, up to a maximum of 2000 mg once daily. If higher doses are required, the total daily dose may be divided into two doses taken with meals. Tablets should be swallowed whole and must not be crushed, chewed, or split. Dose adjustments are necessary in patients with renal impairment, and treatment should be reassessed if eGFR falls below 45 mL/min/1.73 m².

Precautions

  • Renal function should be assessed before initiation and periodically thereafter; avoid use if eGFR <30 mL/min/1.73 m².
