Common use

Doxt SL is indicated for the treatment of severe bacterial infections where broad-spectrum coverage is warranted, particularly in cases involving resistant organisms or mixed flora. Common clinical applications include complicated urinary tract infections, hospital-acquired pneumonia, intra-abdominal infections, severe skin and soft tissue infections, and pelvic inflammatory disease. It is also used in surgical prophylaxis in high-risk procedures and as empirical therapy in febrile neutropenia pending culture results. Its use is generally reserved for situations where first-line agents have failed or are contraindicated.

Dosage and direction

The standard adult dosage is one tablet (Doxt SL) every 12 hours, with or without food, although administration with a meal may reduce the potential for gastrointestinal upset. The tablet should be swallowed whole with a full glass of water; it must not be crushed, chewed, or broken. Duration of therapy typically ranges from 7 to 14 days, depending on the severity of infection and clinical response. In patients with severe hepatic impairment, dosage adjustment may be necessary. For pediatric use (above 8 years and weighing more than 45 kg), consult specific pediatric dosing guidelines—use in younger children is generally avoided due to risk of tooth discoloration.

Precautions

Use with caution in patients with a history of gastrointestinal disease, particularly colitis. Periodic monitoring of renal and hepatic function is advised during prolonged therapy. Avoid excessive exposure to sunlight or artificial UV light due to photosensitivity risk. Use during tooth development (last trimester of pregnancy, infancy, childhood up to 8 years) may cause permanent discoloration of teeth. May cause dizziness or blurred vision—caution when operating machinery or driving. Not recommended for use in pregnancy unless potential benefits outweigh risks.

Contraindications

Hypersensitivity to doxycycline, other tetracyclines, sulbactam, penicillins, or any component of the formulation. Severe hepatic impairment. Children under 8 years of age. Concomitant use with retinoids. Patients with a history of antibiotic-associated colitis.

Possible side effect

Common: Nausea, vomiting, diarrhea, epigastric discomfort, photosensitivity. Less common: Esophagitis, glossitis, stomatitis, vaginal candidiasis. Rare: Pseudomembranous colitis, hepatotoxicity, blood dyscrasias, Stevens-Johnson syndrome, intracranial hypertension. Discontinue immediately if severe diarrhea, skin reactions, or signs of hepatitis occur.

Drug interaction

Antacids containing aluminum, calcium, or magnesium; iron preparations; and bismuth subsalicylate may impair absorption—administer Doxt SL 2 hours before or 4 hours after these products. Barbiturates, carbamazepine, and phenytoin may decrease doxycycline levels. May potentiate the effect of oral anticoagulants—monitor INR closely. Concurrent use with methoxyflurane may cause fatal renal toxicity. Avoid concomitant administration with bacteriostatic antibiotics.

Missed dose

If a dose is missed, take it as soon as remembered unless it is almost time for the next dose. Do not double the dose to make up for the missed one. Maintain the regular dosing schedule to ensure consistent antibiotic levels.

Overdose

Symptoms may include severe nausea, vomiting, and diarrhea. Management is supportive; gastric lavage may be considered if ingestion was recent. Hemodialysis does not significantly remove doxycycline or sulbactam. There is no specific antidote.

Storage

Store in the original packaging below 25°C. Protect from light and moisture. Keep out of reach of children. Do not use after the expiration date printed on the packaging.
