Combimist L Inhaler is a prescription-only metered-dose inhaler designed for the management of asthma and chronic obstructive pulmonary disease (COPD). It combines two active ingredients—Levosalbutamol and Ipratropium Bromide—to provide synergistic bronchodilation, addressing both immediate bronchospasm and underlying cholinergic activity. This formulation is engineered for patients requiring enhanced control over respiratory symptoms, offering a reliable option for maintenance and preventive therapy under medical supervision. Its optimized delivery system ensures efficient pulmonary deposition, maximizing therapeutic efficacy while minimizing systemic exposure.

Common use
Combimist L Inhaler is indicated for the treatment and prevention of bronchospasm associated with reversible obstructive airway diseases, such as asthma and COPD. It is commonly prescribed for patients who have not achieved adequate control with a single bronchodilator or who experience symptoms refractory to short-acting beta-agonists alone. It may be used as a maintenance therapy to manage chronic symptoms and prevent exercise-induced bronchospasm, as well as for acute symptom relief in appropriate clinical contexts.
Dosage and direction
The typical dosage for adults and adolescents over 12 years is two inhalations (puffs) four times daily. Dosage may be adjusted based on severity and individual response, not to exceed 12 inhalations in 24 hours. Shake the inhaler well before each use. Exhale fully away from the mouthpiece, place lips firmly around it, and inhale slowly and deeply while pressing the canister. Hold breath for 10 seconds, then exhale slowly. Wait at least one minute before the second inhalation, if prescribed. Rinse mouth with water after each use to reduce the risk of oropharyngeal side effects.
Precautions
Use with caution in patients with cardiovascular disorders (e.g., hypertension, arrhythmias, ischemic heart disease), diabetes, hyperthyroidism, narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction. Monitor for paradoxical bronchospasm; discontinue immediately if it occurs. Avoid contact with eyes. Not recommended as the sole therapy for acute, severe episodes where intensive measures are required. Periodic assessment of respiratory function and symptom control is advised. Pregnancy and lactation: use only if clearly needed and under medical supervision.
Contraindications
Hypersensitivity to levosalbutamol, ipratropium bromide, atropine, or any component of the formulation. Contraindicated in patients with a history of tachyarrhythmias. Not for use in children under 12 years of age. Avoid in patients with known hypersensitivity to soy lecithin or related food products.
Possible side effects
Common side effects may include headache, dry mouth, cough, nervousness, dizziness, nausea, and throat irritation. Less frequently, palpitations, tachycardia, tremors, blurred vision, urinary retention, and paradoxical bronchospasm may occur. Allergic reactions such as rash, angioedema, and anaphylaxis are rare. Ocular effects like blurred vision or acute narrow-angle glaucoma may occur if the mist contacts the eyes. Report any persistent or worsening symptoms to a healthcare provider.
Drug interaction
May interact with other beta-adrenergic agents (additive cardiovascular effects), anticholinergic drugs (increased anticholinergic side effects), monoamine oxidase inhibitors (MAOIs), tricyclic antidepressants (potentiation of cardiovascular effects), diuretics (hypokalemia risk), and xanthine derivatives. Concomitant use with other sympathomimetics may increase the risk of adverse effects. Caution is advised when using with QT-prolonging agents. Always inform your physician of all medications, including over-the-counter and herbal products.
Missed dose
If a dose is missed, take it as soon as remembered, unless it is nearly time for the next dose. Do not double the dose to make up for a missed one. Resume the regular dosing schedule. Consistent use is important for optimal control; consider setting reminders if doses are frequently forgotten.
Overdose
Overdose may manifest as exaggerated pharmacological effects: tachycardia, angina, hypertension or hypotension, tremors, nervousness, headache, dry mouth, nausea, and dizziness. Severe overdose could lead to hypokalemia, seizures, or arrhythmias. Treatment is supportive and symptomatic; cardioselective beta-blockers may be considered but with caution in patients with bronchospastic disease. Seek immediate medical attention if overdose is suspected.
Storage
Store at room temperature (15–30°C), away from direct heat, sunlight, and moisture. Do not puncture, incinerate, or expose to temperatures above 50°C. Keep the canister away from open flame or heat source during use or storage. Always replace the cap after use. Keep out of reach of children and pets. Discard the inhaler when the dose counter reads zero or after 3 months from first use, whichever comes first.

Reviews
“As a pulmonologist with over 20 years of experience, I find Combimist L to be an effective option for patients with moderate to severe COPD who need dual bronchodilation. The combination offers both rapid relief and sustained control, and most of my patients report improved symptom management and fewer exacerbations.” — Dr. A. Sharma, MD
“I’ve been using Combimist L for my asthma for six months now. It works faster and lasts longer than my previous inhaler. I still carry a rescue inhaler, but I need it much less often. The dose counter is helpful for tracking usage.” — R. Thompson, patient
“From a clinical pharmacy perspective, the adherence benefits of a combination product are clear. However, patients should be counseled on proper inhaler technique and reminded about rinsing the mouth to avoid thrush or hoarseness.” — L. Chen, PharmD
