CiproDex Ophthalmic Solution is a sterile, topical ophthalmic suspension combining the potent antibacterial efficacy of ciprofloxacin, a fluoroquinolone, with the powerful anti-inflammatory action of dexamethasone, a corticosteroid. This synergistic formulation is specifically engineered to treat bacterial ocular infections while simultaneously managing the associated inflammatory response. Common use
CiproDex Ophthalmic Solution is indicated for the treatment of corneal ulcers and the treatment of blepharitis, blepharoconjunctivitis, and bacterial conjunctivitis, where the inflammatory component is a significant factor in the disease presentation. It is particularly valuable in cases where the risk of tissue damage from inflammation is high.
Dosage and direction
For corneal ulcers: Instill 2 drops into the affected eye every 1 to 2 hours while awake for the first 2 days. For days 3 through 9, instill 2 drops every 4 hours while awake. For days 10 through 14, or until resolution, instill 2 drops every 6 hours while awake. For other indicated infections: Instill 1 to 2 drops into the conjunctival sac of the affected eye(s) every 4 to 6 hours for the first 2 days. Based on clinical response, the dosing may be tapered to 1 to 2 drops every 6 to 8 hours for the remainder of the treatment course. Treatment typically should not exceed 14 days. Shake the bottle well before each use. To prevent contamination, do not touch the dropper tip to any surface, including the eye.
Precautions
- Prolonged use may result in overgrowth of non-susceptible organisms, including fungi. If superinfection occurs, discontinue use and institute appropriate therapy.
- Use with caution in patients with a history of sensitivity to other quinolone or corticosteroid preparations.
- As with other antibiotic preparations, prolonged use may result in overgrowth of non-susceptible organisms, including fungi.
- Corticosteroid components may mask, enhance, or activate an ocular infection. They may also increase intraocular pressure (IOP) in susceptible individuals, which can lead to optic nerve damage and defects in the visual fields.
- Posterior subcapsular cataract formation has been reported with prolonged corticosteroid use.
- Not for injection or direct introduction into the anterior chamber of the eye.
Contraindications
CiproDex is contraindicated in patients with:
- Known hypersensitivity to any component of this medication (e.g., ciprofloxacin, other quinolones, dexamethasone, benzalkonium chloride).
- Most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
Possible side effect
The most frequently reported ocular adverse reactions are conjunctival hyperemia, blurred vision, corneal deposits, ocular discomfort, white crystalline precipitate, ocular irritation, eye pain, and eyelid edema. Other ocular reactions include corneal staining, keratitis, allergic reactions, lacrimation increased, photophobia, corneal infiltrates, nausea, and decreased vision. Systemic adverse reactions are rare but possible with topical ophthalmic use.
Drug interaction
While systemic interactions are unlikely with topical administration, patients should inform their physician of all medications they are taking. Concomitant use with other topical ophthalmic medications may potentially lead to physical or chemical interactions. Administer other eye drops at least 5-10 minutes apart from CiproDex.
Missed dose
If a dose is missed, it should be administered as soon as possible. However, if it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not instill a double dose to make up for a missed one.
Overdose
Topical overdose of CiproDex is unlikely to cause life-threatening symptoms. The eye should be flushed with lukewarm tap water. Symptoms of excessive systemic absorption due to accidental ingestion may include systemic corticosteroid and/or antibiotic effects. Symptomatic and supportive care is recommended.
Storage
Store at 2°C to 25°C (36°F to 77°F). Protect from light. Keep the bottle tightly closed when not in use. Discard the bottle within 28 days of opening to prevent contamination. Do not freeze.
