Levlen: Reliable Hormonal Contraception with Cycle Control

Levlen is a combination oral contraceptive pill containing ethinyl estradiol and levonorgestrel, designed to prevent pregnancy with high efficacy when taken as directed. It offers a trusted, reversible method of birth control, with the added benefit of regularizing menstrual cycles and reducing menstrual pain for many users. This monophasic formulation delivers a consistent hormone dose throughout the cycle, simplifying use and supporting predictable bleeding patterns. Healthcare providers often recommend Levlen for its balance of effectiveness, tolerability, and non-contraceptive benefits.

Features

  • Contains 30 mcg ethinyl estradiol and 150 mcg levonorgestrel per active tablet
  • Monophasic formulation for consistent daily dosing
  • 21 active tablets and 7 placebo tablets per blister pack
  • FDA-approved for pregnancy prevention
  • May reduce acne severity in some users

Benefits

  • Over 99% effective at preventing pregnancy when taken correctly
  • Leads to lighter, less painful, and more predictable menstrual periods
  • Can reduce the risk of ovarian cysts and ectopic pregnancy
  • Offers non-contraceptive benefits including improved acne and reduced menstrual cramps
  • Provides reversible contraception without long-term commitment

Common use

Levlen is primarily prescribed for the prevention of pregnancy. It is also used off-label to manage menstrual-related disorders such as dysmenorrhea (painful periods), menorrhagia (heavy menstrual bleeding), and to regulate irregular cycles. Some clinicians may recommend it for the management of moderate acne in women seeking oral contraception.

Dosage and direction

Take one tablet daily at approximately the same time each day, for 28 consecutive days: 21 active tablets followed by 7 placebo tablets. Begin the first pack either on the first day of menstrual bleeding (Day 1 start) or on the Sunday after menstruation begins (Sunday start). Subsequent packs should begin immediately after finishing the previous pack. Use a backup contraceptive method (e.g., condoms) for the first 7 days of the first cycle.

Precautions

Do not smoke while using Levlen, especially if over age 35, as it significantly increases risk of serious cardiovascular events. Regular blood pressure checks are recommended. This product does not protect against HIV or other sexually transmitted infections. Report unusual persistent headaches, visual changes, chest pain, severe leg pain, or jaundice immediately. May influence lipid metabolism and glucose tolerance.

Contraindications

  • History of or current thrombophlebitis or thromboembolic disorders
  • Cerebrovascular or coronary artery disease
  • Known or suspected carcinoma of the breast or estrogen-dependent neoplasia
  • Undiagnosed abnormal genital bleeding
  • History of jaundice with prior hormonal contraceptive use
  • Severe hypertension, diabetes with vascular involvement, or headaches with focal neurological symptoms
  • Known hypersensitivity to any component of Levlen

Possible side effects

Common side effects may include nausea, headache, breast tenderness, bloating, or mood changes—these often subside after 2–3 cycles. Less frequently reported effects include melasma, changes in libido, and vaginal discharge. Serious but rare risks include venous thromboembolism, stroke, myocardial infarction, hepatic adenomas, gallbladder disease, and hypertension.

Drug interaction

Effectiveness may be reduced by drugs that induce hepatic enzymes, including: rifampin, barbiturates, carbamazepine, phenytoin, primidone, topiramate, and some HIV medications. Antibiotics such as ampicillin and tetracyclines may also decrease efficacy. Levlen may alter the metabolism of cyclosporine, theophylline, and tizanidine. Consult a healthcare provider about all concurrent medications.

Missed dose

If one active tablet is missed (24–48 hours late), take it as soon as remembered and continue the schedule. If two consecutive active tablets are missed in Weeks 1 or 2, take two tablets daily for two days, then resume regular schedule; use backup contraception for 7 days. If three or more consecutive active tablets are missed, discard the pack and begin a new pack on the next Sunday; use backup contraception for 7 days. If placebo tablets are missed, they can be discarded.

Overdose

Serious ill effects have not been reported following large doses in adults. Overdose may cause nausea, vomiting, or withdrawal bleeding. There is no specific antidote; treatment should be symptomatic and supportive.

Storage

Store at room temperature (15–30°C or 59–86°F). Keep in the original blister pack and away from moisture and light. Keep out of reach of children and pets.
