Dosage and direction

For Erectile Dysfunction (on-demand): The recommended starting dose is 10 mg taken orally prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on individual efficacy and tolerability. The tablet should be swallowed whole with a glass of water. Sexual stimulation is required for the medication to be effective. The dosing for the on-demand regimen should not exceed one dose in any 24-hour period.

The 5 mg dose can be considered for patients where the 2.5 mg dose is well-tolerated but insufficiently effective.

For Benign Prostatic Hyperplasia (BPH): The recommended dose is 5 mg taken orally once daily at approximately the same time each day.

Dosage adjustments are necessary for patients with renal or hepatic impairment, and for those taking concomitant potent CYP3A4 inhibitors. A healthcare provider must determine the appropriate dosage and regimen.

Precautions

Prior to initiating treatment with Cialis, a thorough medical history and physical examination should be conducted to diagnose ED, identify potential underlying causes, and assess cardiovascular status. Patients should be advised that Cialis offers no protection against sexually transmitted diseases. Use with caution in patients who have conditions that might predispose them to priapism, such as sickle cell anemia, multiple myeloma, or leukemia. Contraindications

Possible side effect

Like all medications, Cialis can cause side effects, although not everybody gets them. The most common side effects (>2%) are headache, dyspepsia (indigestion), back pain, myalgia (muscle aches), nasal congestion, flushing, and limb pain. These are typically transient and mild to moderate in nature. Back pain and myalgia are often dose-related and usually resolve within 48 hours.

Less common but more serious side effects require immediate medical attention. These include:

  • Priapism (a prolonged and painful erection lasting more than 4 hours)
  • Sudden decrease or loss of vision in one or both eyes (NAION)
  • Sudden decrease or loss of hearing, sometimes accompanied by tinnitus and dizziness
  • Signs of a heart attack (chest pain, nausea, sweating, pain spreading to the jaw or shoulder)
  • Signs of a stroke (sudden numbness/weakness, confusion, trouble speaking, vision problems)
  • Allergic reactions (rash, hives, swelling of the face/lips/tongue/throat, difficulty breathing)

Drug interaction

Cialis is primarily metabolized by the CYP3A4 enzyme in the liver. Concomitant administration with drugs that inhibit this enzyme can lead to increased tadalafil exposure and a higher risk of associated adverse effects.

Major Interactions:

  • Nitrates: Absolute contraindication. Concomitant use can cause severe hypotension, syncope, stroke, or myocardial infarction.
  • Alpha-blockers: (e.g., doxazosin, tamsulosin): Potentiates the blood-pressure-lowering effects. A conservative starting dose of Cialis is mandatory, and patients must be hemodynamically stable on their alpha-blocker regimen.
  • Potent CYP3A4 Inhibitors: (e.g., ketoconazole, itraconazole, ritonavir, clarithromycin): Concomitant use requires a dose limitation of Cialis. For example, the maximum recommended dose is 10 mg no more than once every 72 hours when taken with ketoconazole or itraconazole.

Missed dose

For the on-demand (PRN) regimen: This regimen is not scheduled. A dose is only taken prior to anticipated sexual activity. Therefore, the concept of a “missed dose” does not apply. Do not take more than one dose in a 24-hour period.

For the once-daily (QD) regimen: If a dose is missed, it should be taken as soon as remembered on that same day. However, if it is not remembered until the next day, the patient should skip the missed dose and take only the next scheduled dose at the usual time. Do not take a double dose to make up for a missed one.

Overdose

In cases of suspected overdose, standard supportive measures should be employed. Given the vasodilatory effects of tadalafil, the primary expected manifestations of an overdose would be related to its cardiovascular actions, including severe and potentially prolonged hypotension, syncope, and tachycardia. Treatment should focus on maintaining cardiovascular support, including placing the patient in the supine position with the legs elevated to help restore blood pressure. Hemodialysis is not expected to accelerate clearance, as tadalafil is highly protein-bound. Gastric lavage may be considered if ingestion was very recent. Management of priapism, if it occurs, is a urological emergency requiring immediate intervention.

Storage

Store Cialis tablets at room temperature between 20°C to 25°C (68°F to 77°F), with excursions permitted between 15°C and 30°C (59°F and 86°F). Keep the medication in its original blister pack or container to protect it from light and moisture. Keep all medications out of the reach of children and pets. Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed.
