The lowest effective dose should be used for the shortest duration necessary to control symptoms. Dose adjustment may be required in elderly patients and those with hepatic impairment.
Precautions
Arcoxia should be used with caution in patients with a history of cardiovascular disease, hypertension, heart failure, or edema. Renal function should be monitored in patients with pre-existing renal impairment, dehydration, or those taking concomitant medications that affect kidney function. Patients with a history of asthma, aspirin-sensitive asthma, or allergic reactions to NSAIDs should avoid Arcoxia. Contraindications
Arcoxia is contraindicated in patients with known hypersensitivity to etoricoxib or any component of the formulation. It must not be used in patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs. Use is contraindicated in patients with active peptic ulceration or gastrointestinal bleeding, severe hepatic dysfunction (Child-Pugh score ≥9), estimated creatinine clearance <30 mL/min, inflammatory bowel disease, congestive heart failure (NYHA Class II–IV), established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease. It is also contraindicated in the third trimester of pregnancy and during breastfeeding.
Possible side effect
Common side effects may include dizziness, headache, edema, hypertension, dyspepsia, abdominal pain, nausea, and fatigue. Less frequently, rash, pruritus, palpitations, fluid retention, and elevated liver enzymes may occur. Serious but rare adverse effects include gastrointestinal ulceration or bleeding, severe cutaneous reactions (e.g., Stevens-Johnson syndrome), hepatotoxicity, acute renal failure, and cardiovascular events such as myocardial infarction or stroke. Patients should seek immediate medical attention if symptoms such as chest pain, shortness of breath, weakness on one side of the body, black stools, or severe skin reactions develop.
Drug interaction
Arcoxia may interact with several medications. Concomitant use with other NSAIDs or aspirin may increase the risk of gastrointestinal adverse events. It may reduce the effectiveness of ACE inhibitors, angiotensin II receptor antagonists, and diuretics, and can increase lithium and methotrexate levels. Co-administration with warfarin or other anticoagulants may enhance anticoagulant effects and increase bleeding risk. Use with cyclosporine or tacrolimus may increase nephrotoxicity. Rifampicin and other CYP3A4 inducers may decrease etoricoxib plasma concentrations. Healthcare providers should review all concomitant medications before initiation.
Missed dose
If a dose is missed, it should be taken as soon as remembered, unless it is almost time for the next scheduled dose. In that case, the missed dose should be skipped, and the regular dosing schedule resumed. Doubling the dose to make up for a missed one is not recommended.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, gastrointestinal bleeding, hypertension, acute renal failure, or respiratory depression. There is no specific antidote for etoricoxib overdose. Management is supportive and symptomatic, including gastric lavage or activated charcoal (if presented early), and monitoring of vital signs, renal function, and electrolyte balance. Hemodialysis is not likely to be effective due to high protein binding.
Storage
Store Arcoxia tablets at room temperature (15–30°C), in a dry place, protected from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
