Alesse: Effective Hormonal Contraception with Low-Dose Estrogen

Alesse is a trusted combination oral contraceptive pill designed to prevent pregnancy with a well-tolerated, low-dose hormonal formulation. It contains a blend of ethinyl estradiol and levonorgestrel, providing reliable contraception while minimizing estrogen-related side effects for many users. This monophasic pill delivers a consistent hormone level throughout the cycle, supporting both efficacy and user convenience. Approved by health authorities, it remains a cornerstone in reversible birth control options.

Features

  • Contains 0.10 mg levonorgestrel and 0.02 mg ethinyl estradiol per active tablet
  • Monophasic formulation for consistent daily dosing
  • 21-day active pill regimen followed by a 7-day placebo interval
  • Small, easy-to-swallow tablets with clear packaging
  • Manufactured under strict pharmaceutical quality controls

Benefits

  • Highly effective at preventing pregnancy when taken correctly, with a Pearl Index of approximately 0.3–2.7
  • May lead to lighter, less painful menstrual periods and reduced menstrual cramps
  • Can improve acne in some individuals due to anti-androgenic effects of the progestin component
  • Offers non-contraceptive benefits such as reduced risk of ovarian cysts and ectopic pregnancy
  • Provides a reversible and user-controlled method of contraception
  • Low estrogen dose may reduce the risk of estrogen-related side effects like nausea or breast tenderness

Common use

Alesse is primarily prescribed for the prevention of pregnancy. It is suitable for individuals seeking a reversible, daily oral contraceptive method. It may also be used off-label for management of moderate acne in females ≥15 years of age who have no known contraindications to hormonal contraception, desire contraception, have achieved menarche, and are unresponsive to topical acne treatments.

Dosage and direction

Take one tablet daily at approximately the same time each day, for 21 consecutive days, followed by 7 days of placebo tablets or no tablets. Menstruation usually begins during the placebo week. Begin the first pack either on the first day of menstrual bleeding (Day-1 start) or on the Sunday after menstruation begins (Sunday start). Follow package instructions carefully regarding missed pills. Use a backup contraceptive method (e.g., condoms) for the first 7 days of the first cycle.

Precautions

  • Not protective against sexually transmitted infections (STIs); barrier methods are recommended for STI prevention.
  • Smoking increases the risk of serious cardiovascular side effects, especially in women over 35 years of age.
  • May influence lipid metabolism, blood pressure, and glucose tolerance; periodic monitoring may be advised.
  • Use with caution in patients with conditions that may be exacerbated by fluid retention.
  • Reduced efficacy may occur with gastrointestinal disorders such as vomiting or diarrhea.

Contraindications

  • History of or current thrombotic disorders (e.g., deep vein thrombosis, pulmonary embolism)
  • Cerebrovascular or coronary artery disease
  • Known or suspected carcinoma of the breast or other estrogen-dependent neoplasia
  • Undiagnosed abnormal genital bleeding
  • Liver tumors, active liver disease, or severe hepatic impairment
  • Known or suspected pregnancy
  • Hypersensitivity to any component of the product

Possible side effects

Common side effects may include:

  • Nausea
  • Headache
  • Breast tenderness
  • Irregular bleeding or spotting
  • Mood changes
  • Weight changes

Serious but rare side effects require immediate medical attention:

  • Sudden severe headache, vomiting, dizziness, or vision changes

Drug interaction

Alesse may interact with:

  • Antibiotics (e.g., rifampin)
  • Anticonvulsants (e.g., carbamazepine, phenytoin)
  • Antifungals (e.g., griseofulvin)
  • HIV medications (e.g., ritonavir)
  • St. John’s Wort
  • Certain seizure and migraine medications

These agents may reduce contraceptive efficacy. Additional non-hormonal contraception is advised during and for 28 days after concomitant use.

Missed dose

Refer to the patient information leaflet for detailed instructions:

  • If one active pill is missed (24–48 hours late): Take it as soon as remembered, and continue the pack as scheduled. No backup needed.
  • If two active pills are missed in Weeks 1 or 2: Take two pills on the day remembered, two the next day, then resume daily dosing. Use backup contraception for 7 days.
  • If three or more active pills are missed: Discard the pack, begin a new pack on the next Sunday, or restart a new pack the same day. Use backup contraception for 7 days. Consider emergency contraception if unprotected intercourse occurred.

Overdose

Serious ill effects have not been reported following acute overdose. Possible symptoms may include nausea, vomiting, and withdrawal bleeding. There is no specific antidote; treatment should be symptomatic and supportive.
