Aciphex (rabeprazole sodium) is a proton pump inhibitor (PPI) designed to provide long-lasting relief from gastroesophageal reflux disease (GERD) and other acid-related disorders. By targeting the final step of gastric acid production, Aciphex offers potent and sustained suppression of acid secretion, promoting esophageal healing and symptom resolution. Its delayed-release formulation ensures optimal delivery to the site of action, making it a trusted choice among healthcare professionals for managing chronic and severe acid-related conditions.
Features
- Active ingredient: Rabeprazole sodium
- Available in 20 mg delayed-release tablets
- Designed for once-daily dosing
- Enteric coating to protect against gastric degradation
- Rapid onset of action with effects lasting up to 24 hours
- Suitable for short-term and maintenance therapy
Benefits
- Provides rapid and sustained relief from heartburn and acid regurgitation
- Promotes healing of erosive esophagitis
- Reduces the risk of GERD symptom recurrence
- Minimizes nocturnal acid breakthrough
- Supports improved quality of life for patients with chronic acid-related symptoms
- Compatible with long-term management strategies under medical supervision
Common use
Aciphex is commonly prescribed for the treatment of gastroesophageal reflux disease (GERD), including symptomatic relief and healing of erosive esophagitis. It is also used in the management of duodenal ulcers, gastric ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. Additionally, Aciphex may be utilized as part of combination therapy for Helicobacter pylori eradication.
Dosage and direction
The typical adult dosage for GERD and healing of erosive esophagitis is 20 mg once daily for 4 to 8 weeks. Maintenance therapy may involve 20 mg once daily. For H. pylori eradication, Aciphex is usually prescribed as 20 mg twice daily alongside antibiotics for 7 to 14 days. Tablets should be swallowed whole, with or without food, and must not be crushed, chewed, or split.
Precautions
Patients should be advised to inform their healthcare provider of any liver disease or severe kidney impairment prior to starting Aciphex. Long-term use may be associated with an increased risk of bone fractures, vitamin B12 deficiency, and hypomagnesemia. Regular monitoring of magnesium levels is recommended during prolonged therapy. Use during pregnancy only if clearly needed; caution is advised in nursing mothers.
Contraindications
Aciphex is contraindicated in patients with known hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation. Concomitant use with rilpivirine-containing products is contraindicated due to the potential for reduced antiviral efficacy.
Possible side effect
Common side effects may include headache, diarrhea, nausea, vomiting, abdominal pain, and flatulence. Less frequently, patients may experience dizziness, rash, dry mouth, or elevated liver enzymes. Rare but serious adverse effects include Clostridium difficile-associated diarrhea, acute interstitial nephritis, and cutaneous lupus erythematosus.
Drug interaction
Aciphex may alter the absorption of drugs that require gastric acid for optimal bioavailability, such as ketoconazole, iron salts, and digoxin. It can also interact with warfarin, potentially increasing INR. Concurrent use with methotrexate may elevate methotrexate levels. Caution is advised when co-administering with clopidogrel due to potential reduced antiplatelet activity.
Missed dose
If a dose is missed, it should be taken as soon as remembered unless it is nearly time for the next scheduled dose. In that case, the missed dose should be skipped, and the regular dosing schedule resumed. Doubling up on doses is not recommended.
Overdose
Limited data exist on Aciphex overdose. Reported manifestations may include confusion, drowsiness, blurred vision, tachycardia, nausea, and diaphoresis. Management is supportive and symptomatic; dialysis is not likely to be effective due to high protein binding. In case of suspected overdose, seek immediate medical attention.
Storage
Store at room temperature (20–25°C or 68–77°F), with excursions permitted between 15–30°C (59–86°F). Keep in the original container, tightly closed, and protect from moisture and light. Keep out of reach of children and pets.

Reviews
Clinical studies and post-marketing surveillance indicate high patient satisfaction with Aciphex, particularly regarding its rapid symptom relief and tolerability. Many users report significant improvement in quality of life, with reduced frequency and severity of GERD symptoms. Healthcare providers often note its efficacy in healing erosive esophagitis and its favorable safety profile in long-term use.
